Record 'Defects' and allow for relevant reporting

Create a feature for RCA users and Auditors to log “Defects” against a CAR/Worksite. Ensure the Defect is not visible to everyone. And allow Defects to be recorded in such a manner that would allow auditing and reporting of the logged defects in the future (e.g. to answer questions on frequency, work items, seasonality, and more).

To be available for Excavation and Non-Excavation CARs, Global Permits and Events.
